I am confused about what to put for my pension from northern trust company. I only have one form showing monthly amount gross and Year to date. No bixes on this form.

Sounds as though you have a regular pension distribution statement (end of year).  Northern Trust issued 1099-R forms (not sure when, they are not dated).  Envelope looks much like those used for normal distributions.  They are marked as tax documents albeit in pretty much a normal font that can be easily overlooked.  I had to search for same as I missed it when it arrived.

Can I get them to email me another as tax is fue Tuesday. I know gross amount and taxes taken but it is not a 1099 form. I am freaking out that I do not have it.
