I am being asked to provide an elective withdrawal date for my CF 1040 and I don't have a CF 1040

Common Form CF-1040 is a form used by many Michigan cities.  See this Michigan website.

 

Are you filing a Michigan state income tax return?  if so, your Michigan city may have generated CF-1040.  Please clarify.
