How to amend withdrawal of Roth IRA due to excess contribution in 2017. Received 2018 1099-R form.

In 1099-R form:

  • Box 1 and 2a are same amount, $1364
  • Taxable Amount Not Determined "Yes"
  • Total Distribution "No"
  • Codes: J, P

The contributions withdrawn was $1295, but when I went through the questions, I end up owing $522. I thought only earnings are taxed plus 10% penalty for Roth IRA early withdrawal, which should be a lot less.

In other thread, only earnings are entered in 2a, but in another thread, someone says 1099-R form must be entered exactly as received.

It is not clear why the custodian did not do this calculation for you on the form.
My answer is based on the info you provided.

your distribution is 1364 and your taxable amount is $69

The early withdrawal penalty then is $7. You can edit your 1099-R accordingly. and change Yes to No on your second bullet item.
