If those boxes are blank on your 1099-R, leave them blank in TurboTax. Not 0, but blank. These boxes are blank on most 1099-R forms.
If you have boxes appearing in red and can't continue, try tabbing through them to clear the errors.

IF you used that 1099-R form last year, and it transferred over to this year's software.

The easiest solution, is to:
1) scroll to the bottom of the page and click <Back>.
2) Then on the prior page, hit <Continue>
...and this time, all the boxes will be available....but won't require an entry in every dang one...only the ones you need.
