How do I fix an over-payment to my IRAs? I have 2 IRAs - 1 traditional and 1Roth. I mistakenly thought I could contribute a maximum amount ($7000) to each.

How do I properly document withdrawals and notify the IRS of these adjustments?

you must submit a "return of excess and earnings" form to the custodian of your IRA(s).

before Oct 15 to avoid penalty.

If both of these IRA accounts were with the same broker/administrator and they allowed you to make a max contribution to both accounts and did not warn you or stop you then you may want to find another place for your IRA accounts to be housed.
