How do I enter NR4 codes 44 and 49 along with SSA-1099 (3 forms for the same person)

I have US social security, Canada Old Age security Pension and Quebec pension plan.  how do I enter all 3 of these for the same person.

For US income tax purposes, you will enter the income from the Canadian equivalent social security plans in a single figure with US social security income. 

However, you will need to enter any tax withheld through the foreign tax credit section of the program.

What about NR4 for codes 27... these are not social security equivalents... they are 401K equivalents.
