how do i enter my wife's 1099R information. social security 1099R entries can be done on same screen. it seems that 1099R 401R does not.

Yes.  Just enter another 1099R.  It will ask who it is for.

Enter a 1099R under
Federal Taxes
Wages & Income
Then scroll way down to Retirement Plans and Social Security
Then IRA, 401(k), Pension Plan Withdrawals (1099-R) – Click Start
