Here is how my 1099-R is filled out. box 12 state tax withheld-EXEMPT box 13 State payers id-3 box 14 state distribution-NY This doesnt line up to the boxes turbo tax has

every time i leave them blank like it says...it shows that I owe taxes. Can someone please help?

If box 12 is zero then leave  boxes 12, 13 & 14 blank .
