HAVE A 401-K BUT NO CONTRIBUTIONS FOR 2016, SHOULD I UNCHECK THE BOX FOR 1099-R?

Yes. A 1099-R form is a form you receive when you've withdrawn money from a retirement account. Also, the 401-K contributions will always be reported on your W-2.
