Form 1099-R, Field 7 - Distribution Code is 4D. 4 = Death, but I'm not dead and it was a distribution made to me. How do I answer "Tell Us When This Person Was Born?

Somebody died for you to get a distribution with a Code 4D as an inheritance non-qualified annuity. TurboTax needs this to determine how old "This Person" would have been had they lived.”  This would mean that it is asking about the person who died.

The 4 is distribution upon death, in this case to an heir.

 

The D indicates a nonqualified annuity. You will have to pay taxes on the earnings but not the principal. The principal should be in Box 5.
