For the section "Credit for Retirement Savings Contribution" do I list a distribution I made in 2015 from a 401k if I have already claimed it for 2015 and paid the taxes

Yes, for the Retirement Savers Credit purposes, enter your 2015 distribution, even if you paid taxes on it.

You won't be taxed a second time.

That section is for contributions made in 2016, not distributions, and not 2015.

No there's a question asking abt distributions made from 2013 through 2016. This is within the contribution section.
