For the past 3 years, i accrued medical bills with a credit card, of approx 33k . Took out 40k from 401k. Can i say i used all of it to pay bills for 2019?

Unfortunately, no. Medical expenses are deductible in the year paid. If you paid any expenses in 2019, either by cash or credit card, those are the only ones deductible. Anything paid by a credit card is considered paid for that year, whether the card is paid off or not.
