For "Enter Prior Year Roth IRA contributions", do I include contributions from all Roth IRA accounts even if I only withdrew some from one acct?

I have two completely separate Roth IRA accounts.

Yes.  For tax purposes, all of the Roth IRAs owned by an individual are treated as a single account.
