For a 1099-R, box 7 says IRA/SEP/SIMPLE. What distribution code is this (category)?

"IRA/SEP/SIMPLE" is the name of the checkbox adjacent to the code box, not the code box.  If you were age 59½ or over at the time of the distribution, you'll likely find that box 7 has a 7 in it.
