Entering names in the consent to disclose form. Wife has 2 names as her legal first name. The form won't take it. Does this affect legality of form?

On your tax return, you should enter the name exactly as it is shown on your current Social Security card. However,  if your first name is too long or you have two first names, it is acceptable to use the shorten version or just initial of your second name. It won't affect your tax return or create any legal issues.
