Entering all info as presented on 1099-R. Income should be EXEMPT per NYS Retirement plan, yet return is showing $1,124 due ... ???

1099-R form
box 12 - EXEMPT
box 13 - 69-0210001
box 14 - NY
How do I enter this in Turbo Tax? Wont let me type EXEMPT

If no dollar amount in box 12 then  ALL boxes 12-14 MUST be left blank.   (Some payers automatically fill in box 13 & 14 even when not required and they should be blank.   They also violate the 1099-R rules by entering anything in box 12 other that a positive dollar amount greater than 0.)
