Do you have to be OVER 59 1/2 to Direct Rollover (code G in block 7) a 401k account to a Traditional IRA (direct trustee to trustee) to NOT pay income taxes on the money?

No, you can be any age on a direct rollover. This is not a taxable event if it is a traditional IRA and there is no penalty.
