Do I need to amend my 2017 taxes for an excess contribution to a Roth IRA for which I received a 1099-R in 2018?

I over contributed to my Roth IRA by $600 for tax year 2017 (contribution was made in 2017).  I had the excess removed on March 23, 2018 before filing deadline.  I received a check for $672.36, the statement indicated $600 was excess distribution, $64.40 was attributable earning, $7.24 was Federal withholding, $0.72 was state withholding. This year I received the 1099-R (year 2018) with Box 1: $672.36 and 2a: $72.36 and Box 4 $7.24 and Box 7: P, J and Box 12 $0.72.
