Do I need a 1099R if I have elective deferrals to my 401k?

A 1099-R is issued and entered only when you take money out of your 401k.

Contributions to your 401k are shown and entered with your W-2 (box 12) and nowhere else in the program.

No, you do not need a 1099-R since you are putting money into your 401K.

The 1099-R is issued only when you take money out of your 401K account.
