Do I include my rollover IRAs in the value of all traditional IRA accounts for form 5498? This rollover account had been an 401k prior to 2015

Yes.  If TurboTax is asking for the year-end value of your traditional IRAs, you must enter the total of all of your traditional IRAs, including rollover, SEP and SIMPLE IRAs.
