Can I complete 2 separate SSA-1099 forms because I received Survivor benefits part of the year then went to benefits under my own SS#?

Do I add the info in my 2 forms together then in TT just make one SSA-1099 form report?

If they are under the same social security number you can add them together.

Yes if they are both for you with your ssn on them.  Then add them together first and enter the total.
