If you have a 401k through your employer, it was entered already when you entered information from the boxes on your W-2.  You do not enter it again.  Or...are you self-employed?

No.  401(k) contributions aren't the same as an IRA, and they shouldn't be entered on your tax return.  The deduction for traditional 401(k) contributions has already been accounted for in your W-2.

No. Your 401(k) contributions are Not for an IRA.

Your 401(k) contributions are reported on your W-2 in box 12 with a code of D.  Those contributions are not entered anywhere else on a tax return.
