Can a qcd be made from a 401k or just an ira?

when entering a 1099-r for an ira, questions are asked about a QCD. What about a 1099-r for a 401k...can a 401k distribution to charity qualify as a QCD?

A QCD cannot come from a 401(k) account.  Only from an IRA.
