on the 1099r it has distribution code 1 in it, was forced to stay home several weeks unpaid after using up leave and made the withdrawal to keep up on bills.

Since the distribution was made in 2021, it will not qualify for the COVID related disaster relief, as that provision expired at the end of 2020.
