As a government annuitant who gets a 1099-R, where do i find the simplified form to calculate the taxable income?

I am not sure where to get a copy of the "simplified form" that would let me calculate the taxable amount of my annuity?

When you enter your 1099-R into TurboTax, you will be asked questions about your annuity in the interview.  The 'Simplified Method' is generally used after 1996.  

If you want to be able to view your forms and make entries/changes directly on forms, you may want to use the Desktop TurboTax product.
