Are firefighter pensions taxable on federal level? My state does not. If so, would I use simplified method to determine amount since none is in box 2 on 1099-R form?

Yes, they are taxable on your federal income tax return. If some of your pension contributions were previosly included in your income, it is customary to use the simplified method to determine the portion of pension distributions that are not taxable in the current year.
