It looks like one of your 1099-R entries (retirement distributions) has a blank zip code entered for the plan administrator or the IRA administrator.

Go to Federal->Wages & Income->Retirement Plans and Social Security and click on Start or Revisit (or Edit) for "IRA, 401(k), Pension Plan Withdrawals (1099-R)"

The next screen should be entitled "Your 1099-R Entries". There should be a table with a row for each 1099-R that you entered.

Since your 1099-R is called "untitled", it makes me think that you accidentally created a 1099-R without meaning to. Look to see if there is an entry with no name under PAID BY. You can see that this is the row is blank, but there is "Edit" and "Delete" to the right.

If so, delete it, and continue with your review. Otherwise, open each 1099-R and look at the zip code.
