5329-T - where do I get one of these?

I guess I contributed to an HSA and TurboTax is asking me to enter info from a form I don't have. Not sure what to do.


"The value of your HSAs on December 31, 2016. Include any contributions made after December 31, 2016 that were for 2016." I don't know what that number is but know what the current value of the HSA is.

A 5329 is the form that calculates the penalty that you might owe from haveing excess contributions to your HSA.

You don't receive one, instead TurboTax creates one for you if you need it.

The value of your HSA on December 31, 2016 will be found on your 5498-SA that you may or may not have received yet.

I say "may or may not have received", because the form is not due to you until May 31st. This odd date is due to the fact that you can contribute to your 2016 HSA until April 18th.

However, many banks have already mailed them out so you may have it already. If not, just call your HSA plan administrator and ask them for a copy.
