401k termination benefit for a small amount. Bank didn't send official form but sent statement with taxable amount. Where do I put it in Turbotax?

My wife started working at a contract house in 2017, but decided to open a 401k with them until late that year.  Early 2018, she got a permanent job offer at a new company but that company couldn't/wouldn't transfer the 401k over.  Mid-late 2018, the 401k bank sent a "Termination Benefit" back of the amount of ~$110 and listed it as taxable income on the form, but with no official tax statement.  Where do I put it in income in TurboTax?

If you did not receive a 1099-R or a 1099-Misc for this income, you can list it under 'Other Taxable Income'.

Here's how to do that:

Select Federal TaxesWages & IncomeI'll Choose What to Work On.

Scroll to Less Common Income, and select Miscellaneous Income,  Scroll to Other Reportable Income.

YES to Other Taxable Income. Enter a Description and Amount (screenshot).
