401k rolled over to roth IRA

Do I have to continue to report this as income as I did in my 2021 return. I can't use these funds for 5 years

I understand in a prior year, 2021, you rolled over your 401k to a Roth IRA.  In 2021 you were issued a Form 1099R  and reported the rollover on your 2021 tax return.

 

You do not need to report that rollover again, only on the tax return for the year it took place.  When you begin to withdraw the funds, you will be issued a Form 1099R that you will report on your tax return for that tax year.
