2022 1099-R Box 14, 15, 16

In 2022, I was a resident in one state for 7 months (Illinois) and became a resident in another the remaining 5 months (AZ) of 2022.  The 1099-R I received in Box 16 state distribution isn't correct, it basically is flipped between the 2 states.  Also, please keep in mind that no tax or local taxes were withheld for any of the 2 states. What should I do?  Do I enter the amounts reported on the 1099R or do enter in the amount that should have been printed on the form?  Because as we know, the amounts are carried forward from the Federal return to the respective states.  Need some help!!!!

The best thing to do is to ask for a corrected 1099. The IRS gets copies so neither of your scenarios is ideal. Contact the plan admistrator and inform them of their mistake.

Yes, it would be best to ask the plan administrator for a corrected Form 1099-R. This is easier because there were no withholdings, so at least they did not send withholding to the wrong states. However, as you state, the taxable income amounts will carry over into the state returns, so it should be corrected.
