1099-R verify questions do not line up w/ the actual boxes on 1099-R form. For ex. it asks for the state listed in box 13 - box 13 on the 1099 form is "date of payment."

It looks like when TurboTax asked what kind of 1099-R you have, you selected CSA-1099-R or CSF-1099-R. On those forms box 13 is the state. If you have a plain Form 1099-R you have to select "Form 1099-R" as the form that you have. TurboTax will not ask you for box 13 on a plain Form 1099-R.

Thank you Champ for your response, unfortunately that's not the case here. It auto-filled from previous years, but I went back to verify that it was still showing as a 1009-R and it is. I'm so confused. 

If you are asking about the the verify or summary screen after you have entered the 1099-R then the box numbers on that screen have not been updated with the new numbers , but the data is correct.   That will be fixed in a coming update.
