I had a 403b Roth and rolled it over to a Roth IRA. I received the 1099-R form, but it doesn't have the "box 9," so I had to check the "Substitute Form 1099-R" and enter the information that way, even though the form is not in error. Now when I try to file, it tells me I need a "Form 4852: Substitute Form W-2, 1099-R." I'm assuming that part of this is that my work goofed (not surprised) on the W-2 and I need a new one, but in addition, I need a 4852 (?) and additional 1099-R? I spoke to Nationwide and they confirmed that the information on my 1099-R is accurate, and so are the codes in box 7 (listed as BG, but BH would also be accurate). It looks like nobody (?) is allowed to e-file this year from other comments I'm seeing, but am I only waiting on the replacement W-2, or is there some other form I am supposed to be waiting on? First of all, let's start fresh. Go to tax tools>tools>delete a form>delete all 4852 forms that appear in the list of forms. Chances are, you may have more than one.
Box 9 is irrelevant and any employee contribution would be in box 5, but for a code BG which is a direct rollover of a Designated Ruth 401(k) to another Designated Roth 401(k) plan.
If this was rolled into yiur Roth IRA then code H should have been used.
This has nothing to do with a W-2. The form 4852 is used for different things.
You can use a substitute 1099-R with a code BH and explanation statement that it was a Roth IRA destination and the account trustee refused to correct it. Everything else should re the same on the substitute.

As macuser_22 said, code BG reports a rollover to a designated Roth account in a different qualified retirement plan like a 401(k), not to a Roth IRA. If the rollover was actually to a Roth IRA, the code should be H (without code B; code H is not permitted to be combined with code B since code H by itself already says that the distribution if from a designated Roth account in a qualified retirement plan).
Right - I meant to say H for the substitute - I corrected it.
After reading this response, I ended up finding "forms" in the upper right corner of the application. When I clicked it, I scrolled through looking for the "4852" and when I found it, I deleted it. When I went back to the digital "paper" version (instead of the step-by-step version), the 9a and 9b boxes I expected to see were there, when they hadn't been in the "step by step" version. I made sure to unclick the box noting that the 1099-R needed to have a 4852 form generated, and then my tax return showed no errors when it was checked again. I was able to submit electronically. The forms mode is not supported for data input and may not properly add the necessary explanation statements for a substitute 1099-R that are required by the IRS.
Using the forms mode means that you know what forms need to be linked that the step-by-step mode does for you. Filing a substitute 1099-R without the statements will probably get you an audit letter form the IRS.
