1099-R input issues, do I need 4852 if correct and how do I get one?

I had a 403b Roth and rolled it over to a Roth IRA.  I received the 1099-R form, but it doesn't have the "box 9," so I had to check the "Substitute Form 1099-R" and enter the information that way, even though the form is not in error.  Now when I try to file, it tells me I need a "Form 4852: Substitute Form W-2, 1099-R."  I'm assuming that part of this is that my work goofed (not surprised) on the W-2 and I need a new one, but in addition, I need a 4852 (?) and additional 1099-R?  I spoke to Nationwide and they confirmed that the information on my 1099-R is accurate, and so are the codes in box 7 (listed as BG, but BH would also be accurate).  It looks like nobody (?) is allowed to e-file this year from other comments I'm seeing, but am I only waiting on the replacement W-2, or is there some other form I am supposed to be waiting on?  Thank you.

 First of all, let's start fresh. Go to tax tools>tools>delete a form>delete all 4852 forms that appear in the list of forms. Chances are, you may have more than one.

  1. Now review your 1099R
  2. After review, scroll through the section until the reach the area that says Tell us if any of these uncommon situations apply
  3. Here choose I need to file a substitute 1099-R
  4. As you continue through the screens, you will eventually arrive at the screen that says Substitute 1099-R agreement
  5. press continue and answer the questions that is asked. From then on, your 4852 is complete.

Box 9 is irrelevant and any employee contribution would be in box 5, but for a code BG  which is a direct rollover of a Designated Ruth 401(k) to another Designated Roth 401(k) plan.

 

If this was rolled into yiur Roth IRA then code H should have been used.

 

This has nothing to do with a W-2.  The form 4852 is used for different things.

 

You can use a substitute 1099-R with a code BH and explanation statement that it was a Roth IRA destination and the account trustee refused to correct it.    Everything else should re the same on the substitute.

As macuser_22 said, code BG reports a rollover to a designated Roth account in a different qualified retirement plan like a 401(k), not to a Roth IRA.  If the rollover was actually to a Roth IRA, the code should be H (without code B; code H is not permitted to be combined with code B since code H by itself already says that the distribution if from a designated Roth account in a qualified retirement plan).

After reading this response, I ended up finding "forms" in the upper right corner of the application.  When I clicked it, I scrolled through looking for the "4852" and when I found it, I deleted it.  When I went back to the digital "paper" version (instead of the step-by-step version), the 9a and 9b boxes I expected to see were there, when they hadn't been in the "step by step" version.  I made sure to unclick the box noting that the 1099-R needed to have a 4852 form generated, and then my tax return showed no errors when it was checked again.  I was able to submit electronically.  Thank you all for your responses.

The forms mode is not supported for data input and may not properly add the necessary explanation statements for a substitute 1099-R that are required by the IRS.

 

Using the forms mode means that you know what forms need to be linked that the step-by-step mode does for you.    Filing a substitute 1099-R without the statements will probably get you an audit letter form the IRS.
