1099-r correction help

My wife received her 1099-r. It’s saying that it’s taxable even though she moved it to a Roth IRA directly. How do I get this form corrected?

A direct rollover from a traditional 401(k) account, indicated by the code G, to a Roth IRA is indeed taxable.  The Form 1099-R is correct, so enter it as received.

What did she move it from?  Unless it was from another ROTH account then it is taxable.  What code is in box 7?

I believe it states G. And it came from pension provider. I wish they advised us if that was the case

Code G is direct transfer and not taxable.  It probably needed to go into a Traditional IRA not a ROTH IRA.    Still need to know exactly what kind if account it came from.  Like a 401K and if it was a Roth.   @dmertz can you help?
