I am in the US Public Health Service, which is one of the seven uniformed services. Is USPHS included in the Active Duty Military and National Guard Reservist Pay for OR?

Yes, the United States Public Health Service Commissioned Corps (PHSCC) is active duty for Oregon income tax purposes.
