While working for Disneyland I received a share which I’ve received dividends 1099-B . Sold DSPP-Common Stock with no taxes taken out. How do I fill out Cost Basis?

I participated in Disneyland stock as an employee back in 2000-2001. Once I left company I kept the stocks, received dividends but never put anything into it. There wasn’t much.  I sold most of my stock in September of 2019. The 1099-B just gives the date sold, how many shares sold and amount of uncovered proceeds. How do I figure the Cost Basis. The price per share when I sold it was $138.86. Turbo tax is asking if it’s short term or long term—how do I answer this?

Since you kept the stock for more than a year then it is long term holding period ... an exact date is immaterial ... as for the basis ... you will need to figure this out in some way using any records you have available ... if you cannot compute a basis then the basis is ZERO.
