How to report T5008 (form received from Canada) in my US tax filing?

I am trying to report my T5008 on my US tax return and I dont received 1099-B, so how should i enter this info? I wasn't given the option to say that I don't have a 1099-B if i choose Stocks, Bonds, Mutual Funds. Please help.
