Do I need to report a fixed annuity account on my tax forms?  If so, how?  I receive no tax information from the company holding funds.

Hopefully this answer helps you:

If your annuity is within an IRA account, it is not taxable or reportable.

If not, you have no reporting requirement unless you received a distribution from your annuity in 2015 in which case you will need to report a 1099-R tax statement.
