Entering 1099-B info for an ESOP. Chose 'Stock (non-employee)' as investment type. It asks 'How did you receive this investment?' No options listed apply. What now?

you should have a 1099-R for an ESOP.

In which case, your basis on a 1099-B equals your proceeds.
