1099B has no boxes checked except shares sold & total value. R d as a bonus to salary? How do I report as these shares are from 1996?

Di I report value of shares when issued & report  difference as income?

Your acquisition date would be the date you received them, and your cost basis would be the value at the time you received them (assuming that amount was included in your taxable income that year).  The difference will be a long-term capital gain.
