1099-B do not have all columns

Computershare issued a 1099-B but do not have all fields populated - I have only following,

- Applicable check box on Form 8949 has "x" marked

- (1a) has number of units 

- (1c) Date Sold or Disposed: 03/02/2022

- (1d) proceeds have the amount in $
- (4) Federal Income Tax withheld: $0.00

- (5) Check if noncovered Security has "x" marked 

How to fill this in ? It was asking for column (1e) Cost or other basis when i try to manually enter. Note that there is no control number or document ID.

That means the cost basis was not reported to you. You will have to do some research to determine when you acquired the security and at what cost. You can enter $0 for the cost basis, but that means you won't get a deduction for the cost when you report the gain on your tax return.
