You have to keep trying to contact the employer. W-2's are issued by employers, who have until January 31 to issue them to you. You need to make sure the employer has your current address. There are many ways to contact your former employer--email, snail mail, telephone, etc.
Some employers make W-2 's available for import so later in January you could also try to import it.
