What is form CF-1040. I don't know where it is generated from but it is asking for an employer ID for scholarships and grants which does not make sense.

Turbo has automatically generated a W-2 worksheet for "Scholarships and Grants" and is asking for an employer ID for it. I have no idea where this is coming from and I can't find the document itself. Everything in the document is greyed out except the "Employer's ID Number".

CF-1040 is a Michigan City tax return. 

Some Michigan cities, such as Grand Rapids, taxes scholarships. 

 

If you have a Form 1098-T, use the EIN, (Payer ID) (Filer's federal ID no.) listed on the 1098-T for the school and enter that for employer ID.
