What dollar amount do i put in under Original loan balance for a HELOC loan? The balance or the amount that I can borrow up to?

It's your total line of credit - the amount you can borrow up to.  If you have a $100,000 HELOC, your original loan balance is $100,000 - even if you haven't yet drawn on it.
