Is it okay that my maiden name is on my student loan interest form and is there any place on here where I should note it?

You will not have to note it anywhere.  The IRS uses the information from the Social Security Administration (SSA) to match names with Social Security numbers (SSN).  If there is a difference that cannot be verified by the IRS, they will contact you, (via mail, they will not call).  However, if you changed your name with the SSA when you got married, you should have no problem as the IRS will have both names matched to your SSN.
