I did not receive a 1098-T form from my school. I obtained an associates degree and the school was paid for with Army Tuition Assistance. Do I just leave the spots blank?

If you did not get a 1098-T for 2016, there would be nothing to report.

If the tuition assistance paid all the expense, you would not be eligible for an education credit. 

If the tuition assistance was not more than the expense, and you did not receive a refund of the tax-free assistance, there would be no income to report.
