How do I make it clear on my tax forms that I did not pay for my own tuition?

That's not something you need to make clear. It doesn't matter who paid your tuition.  If scholarships paid, there is a place to enter the amount of scholarship.  But there is also away for you (or your parents if you are a dependent) to claim a credit even  if you are on scholarship.

Don't enter the 1098-T.    You can take an education credit even if someone else paid your tuition.  Qualified education expenses paid on behalf of the student by someone other than the student (such as a relative), are treated as paid by the student.
