Form 5674 is from Missouri: Verification Of Rent Paid. This is necessary for 2018 Property Tax Credit Claim in Missouri.

It has to be the form.

 

Your tenant may be entitled to a number of credits but not without that form.  

 

Here is the form.  It's a pretty simple one.

Form 5674, Missouri Verification of Rent Paid, is a form filled out by the landlord and sent to the Missouri Department of Revenue each year.  Form 5674 is not attached to the tenant's return.  You will attach form MO-PTC or MO-PTS and mailed to:

 

Missouri Department of Revenue

PO Box 500

Jefferson City, MO 65105

If you are the tenant and want to report rent paid, you would follow these steps in TurboTax online:

 

  1. Click State Returns
  2. Click Edit Next to Missouri
  3. Click Property Tax Credit to enter the renter's credit

The circuit breaker tax credit is a service that helps seniors and persons with disabilities apply for property tax credits or rent rebates.  To be eligible for the credit, you must meet the following requirements:

 

  • 65 years of age or older or
  • a person 18-64 who receives SSI, SSD, or Veterans Disability, or
  • 60 and older and receiving Surviving Spouse benefits from SSA

 

You will enter the MO-PTS following the same steps as above:

 

  1. Click State Returns
  2. Click Edit Next to Missouri
  3. Click Property Tax Credit to enter the renter's credit

As a reminder, form 5674 is a form that will be filled out by your landlord and is not attached to your return.  The form will need to be completed by the landlord and mailed to the following address:

If you're trying to claim the renter's credit, you should not need your rebate paper from 2018.  Your landlord would have been responsible for mailing form 5674 yearly to the Missouri Department of Revenue.  You can follow up with your landlord to verify they filled out the form regarding your rent.  

 

To claim the credit you can follow these steps:

 

  1. Click State Returns
  2. Click Edit Next to Missouri
  3. Click Property Tax Credit to enter the renter's credit
