I worked for Uber Eats as a delivery driver in 2019; from Jan-Apr as me then started an LLC in May. Do I separate them out when filing?

single member or multimember LLC? doing the same business? Does the LLC have an EIN? Has the LLC elected to be taxed as a corporation? Why did you start the LLC?
