I have a small homebased business (LLC) that is on a Indian Reservation (my home), how do I answer: Assets on a reservation or in a disaster area?

Valid responses are Yes/No.  I do not live in a disaster area.

You would answer yes. You live on an Indian Reservation and should indicate that yes, you do live there.
